2. Key Fields Explained
- Stakeholder Name: The name of the individual who received the feedback survey (either employee or client/partner).
- Role (Employee/Client): Indicates whether the respondent is an internal employee or an external client/partner. This helps track the different groups separately.
- Survey Sent Date: The date when the feedback survey was initially sent to the stakeholder. This helps to track when the survey invitations were distributed.
- Response Received: Indicates whether a response has been received (Yes or No). This helps identify who has submitted their feedback.
- Follow-up Required (Yes/No): Tracks whether a follow-up reminder is needed for those who haven’t submitted their feedback yet. If “Yes,” a follow-up email or reminder should be sent.
- Notes: Additional notes can be added here, such as when reminders were sent, specific comments received, or if there are any issues or special circumstances related to that stakeholder’s response.
3. Instructions for Use:
- Add Stakeholder Details:
