To ensure consistency, accountability, and high-quality reporting in the SayPro Monthly Monitoring and Evaluation (M&E) process, all employees involved in community-based programs and fieldwork are required to submit the following documents. These documents help track progress, measure impact, and contribute to strategic decision-making across SayPro initiatives.
1. Monitoring Plan
Purpose:
Outlines the structure and approach for conducting monitoring and evaluation activities.
Must Include:
- Objectives of the monitoring initiative
- Methodology (qualitative, quantitative, or mixed-method)
- Data collection tools (surveys, interviews, focus groups, observation, etc.)
- Timeline and frequency of monitoring activities
- Key Performance Indicators (KPIs) and how they will be measured
- Roles and responsibilities of team members
- Ethical considerations and community consent processes (if applicable)
2. Field Data Collection Tools
Purpose:
Ensure consistent, accurate, and reliable data collection across programs and regions.
Must Include:
- Standardized survey forms, questionnaires, or interview guides used during fieldwork
- Observation checklists or focus group templates
- Digital data capture tools (where applicable) including app or spreadsheet formats
- Notes on any language translations or adaptations made for community relevance
- Documentation of ethical compliance (e.g., informed consent forms)
Submission Deadline: Alongside data reports, within 3 working days after fieldwork completion
3. Monthly M&E Progress Report
Purpose:
Provide a comprehensive update on program performance, data collected, and progress toward KPIs.
Must Include:
- Summary of activities conducted during the reporting period
- Progress against targets and indicators (with data and visuals where applicable)
- Challenges encountered and mitigation strategies
- Beneficiary feedback or case examples
- Preliminary recommendations based on field observations
- Any deviations from the monitoring plan
Submission Deadline: On or before the 28th of each month
4. Stakeholder Engagement Summary
Purpose:
Capture feedback, perspectives, and participation levels of key community stakeholders involved in or affected by the program.
Must Include:
- Summary of meetings, consultations, or interviews with local stakeholders (e.g., community leaders, local government, partner NGOs)
- Key themes or concerns raised by stakeholders
- Feedback from beneficiaries or community representatives
- Stakeholder recommendations for improving program delivery
- Any follow-up actions taken or planned
Submission Deadline: Within one week following stakeholder engagements
5. Program Adjustment Proposals (If Applicable)
Purpose:
Recommend evidence-based changes to ongoing programs for improved impact or efficiency.
Must Include:
- Clear description of the issue or gap identified
- Supporting data from monitoring findings
- Suggested adjustments to program design or delivery
- Anticipated impact of the proposed change
- Resource or support needs (if any) to implement the adjustment
- Risk assessment or potential challenges
Submission Deadline: As needed, but no later than the end of the monthly reporting period
Submission Guidelines
- All documents must be submitted through the SayPro Research Submission Portal or designated shared folders.
- File names must follow the format:
ProgramName_Location_DocumentType_EmployeeName_MonthYear
Example: YouthSkills_ECape_ProgressReport_NSmith_Apr2025 - Late or incomplete submissions may impact program evaluations and team reporting efficiency.
