  3. Reported By: Indicate who reported the issue (can be system users or team members).
  4. Issue Description: Provide a detailed description of the issue, including any relevant symptoms or patterns.
  5. Priority: Classify the issue based on its severity: High (critical), Medium (affects some functionality), or Low (minor impact).
  6. Status: Track the issue’s progress: Open (unresolved), In Progress (being worked on), or Resolved (fixed).
  7. Assigned To: Indicate who is responsible for resolving the issue (usually an IT team member or developer).
  8. Date Resolved: Record the date when the issue was successfully resolved.
  9. Resolution Details: Describe how the issue was fixed or what workaround was applied.
