Training Workshop by SCMR
Purpose:
This document outlines the necessary documentation that employees must submit for participation in the SayPro Monthly February Government Department and Municipality Quotation Submission Training Workshop by SCMR.
1. Proof of Identity
All employees are required to submit a valid government-issued identification document as proof of identity. Acceptable forms of ID include:
- National Identity Card (ID)
- Passport (Valid and up to date)
- Driver’s License (If applicable)
- Other official government-issued ID
The ID document should be clear, legible, and not expired.
2. Proof of Employment
Employees must submit a document verifying their employment with SayPro. This may include:
- Employment Confirmation Letter signed by HR or a designated company official
- Recent Pay Slip (within the last three months)
- Company ID Card (if issued)
3. Tax and Banking Details
Employees participating in financial transactions or reimbursements related to the training workshop must provide:
- Tax Clearance Certificate or SARS Tax Number
- Bank Confirmation Letter from the bank (not older than three months)
- Proof of Banking Details (e.g., bank statement showing name and account details)
4. Proof of Residence
To verify the employee’s address, one of the following must be submitted:
- Utility Bill (water, electricity, or rates bill not older than three months)
- Lease Agreement (if renting)
- Affidavit Confirming Residence (if no formal proof is available)
5. Educational and Professional Qualifications
For employees requiring certification for the training, the following documents may be required:
- Highest Educational Qualification Certificate
- Professional Certifications relevant to the training workshop
- Proof of Previous SCM Training (if applicable)
6. Declaration and Consent Forms
Employees must complete and submit the following:
- Consent Form allowing SayPro to verify provided documents
- Code of Conduct Agreement confirming compliance with SayPro policies
- Acknowledgment of Training Participation
7. Additional Requirements for Government and Municipality Quotation Submission
Employees involved in the quotation submission process for government departments and municipalities must also provide:
- Company Registration Documents (if applicable)
- B-BBEE Certificate (if applicable)
- Supplier Registration Confirmation with relevant government bodies
- SCM (Supply Chain Management) Training Certificate
Submission Guidelines
- All documents must be submitted via email to the SayPro HR Department before the designated deadline.
- Documents should be in PDF format and labeled appropriately (e.g., “John_Doe_ID.pdf”).
- Hard copies may be required upon request.
