Objective: Gather honest feedback from participants to assess the relevance, clarity, and impact of the M&E system training—and identify areas for improvement.

📝 Survey Design & Content

1. Survey Format

  • Tool: Google Forms, Microsoft Forms, or SurveyMonkey
  • Type: Anonymous (recommended to encourage honest responses)
  • Length: 10–12 questions (takes ~5 minutes to complete)

2. Sample Survey Questions

Section A: Training Content & Delivery

  • The training objectives were clearly defined. (Strongly Agree – Strongly Disagree)
  • The content was relevant to my work. (Yes/No/Somewhat)
  • The pace of the session was appropriate. (Too fast / Just right / Too slow)
  • The trainer(s) explained the material clearly. (1–5 rating)
  • What did you find most useful in the training? (Open-ended)

Section B: System Confidence

  • I feel confident using the M&E system after the training. (1–5 rating)
  • I understand how to enter data, generate reports, and upload evidence. (Yes/No/Somewhat)

Section C: Overall Experience

  • Overall, how satisfied were you with the training? (1–5 stars)
  • What improvements would you suggest for future training sessions? (Open-ended)

📤 Distribution Strategy

1. Email Distribution

  • Timing: Send within 24–48 hours after each training session.
  • Subject Line: We’d Love Your Feedback: SayPro M&E Training Survey
