Purpose:
The SayPro Evaluation Feedback Form (EFF-1) is designed to collect structured feedback from participants, stakeholders, and team members following key activities, events, or interventions. This form is a critical tool for assessing the effectiveness of SayPro’s initiatives, identifying areas for improvement, and capturing lessons learned.
Key Functions of the EFF-1:
- Participant Insight Collection: Gathers qualitative and quantitative data from stakeholders to evaluate the success of projects, activities, or events.
- Performance Evaluation: Helps assess how well SayPro’s activities meet pre-defined objectives, including participant satisfaction, knowledge gained, and perceived impact.
- Improvement Identification: Identifies strengths and areas for improvement in project design, execution, and delivery.
- Feedback for Reporting: Serves as an input for SayPro Monthly Summary Logs (MSL-1), providing data to demonstrate responsiveness to stakeholder needs and programmatic adjustments.
- Continuous Learning: Promotes an organizational culture of reflection and continuous improvement based on feedback.
Template Structure of SayPro EFF-1:
- Activity/Event Information
- Project Name/Activity Title: The name of the project, activity, or event being evaluated.
- Date of Activity: The date when the activity took place.
- Location: Where the activity or event was held.
- Target Audience: Who participated in the activity (e.g., beneficiaries, stakeholders, team members).
- Prepared By: Name of the person or team responsible for collecting feedback.
- Feedback Questions:
- Rating Scale Questions (1-5 or 1-10 scale)
- On a scale of 1 to 5 (or 1 to 10), how would you rate the overall quality of this activity?
- How satisfied were you with the content/agenda of the event?
- How effective was the facilitator/presenter?
- How relevant was the information presented to your needs or interests?
- How likely are you to apply what you learned in your work or life?
- Open-Ended Questions
- What aspects of the activity did you find most beneficial?
- What could have been improved in the delivery of the activity?
- Were there any challenges or obstacles that impacted your experience?
- Do you have any recommendations for improving future activities or events?
- Demographic Information (optional):
- Role or affiliation (e.g., beneficiary, project staff, external stakeholder)
- Any other relevant demographic or contextual information (e.g., sector, region).

Submission and Review Process:
- Feedback Collection:
- The SayPro Evaluation Feedback Form (EFF-1) is distributed at the end of each activity or event, either physically or electronically via the SayPro Monitoring System (SMS).
- Participants are encouraged to complete the form within a set time frame (usually 2-3 days post-event) to ensure timely feedback.
- Data Analysis and Reporting:
- Feedback is aggregated and analyzed by the SayPro Monitoring and Evaluation Office to identify patterns, strengths, and areas for improvement.
- A summary of the findings is prepared and used in monthly and quarterly reports (e.g., SCLMR-1, QSRR).
- Actionable Insights:
- Actionable feedback is used to adapt future activities, training, and project designs.
- Recommendations may also inform strategic changes or improvements to SayPro’s programs.
- Storage and Access:
- The completed forms are securely stored in the SayPro Learning Database.
- Access to feedback reports is granted to key stakeholders, project leads, and partners involved in program improvement and planning.
